Transitional care services refer to our agency CNA’S assisting with the coordination and continuity of care during a movement from one healthcare setting to our client home. We work with health care practitioners to ensure seamless transition in executing of our client care plan.

Our Caregivers are Memory Care Certified
Our caregivers have advanced dementia care training, which is a requirement to service our clients with dementia. We understand the importance of redirecting clients in order to support and keep them safe. Guardian has partner with The Alzheimer’s association in order to stay current with relevant training, knowledge and the skill set necessary to support the family through this journey.
STAY CONNECTED
The Virtual Family Room is a great way for POA, discharge managers, social workers, and the client physician to STAY CONNECTED to our client's care plan. The client provides access to whom they want involved in monitoring their care plan activities, care partner shifts and the daily tasks provided by our agency. Our Virtual Family Room provides a client centered approach that fosters trust and transparency between our agency and client.
