Care & Safety Assessments Service

In order to properly assess and support our clients' needs, we highly recommend an in-home assessment before determining the appropriate care and service rate. Our assessment service is provided with no obligation to initiate services with our agency. Assessments are designed to offer our clients peace of mind and provide resources to help them make informed decisions regarding the best services for their needs.

Our care assessment includes, but is not limited to:

• Home safety assessment
• Eating with purpose meal assessment
• Virtual Family Room so the families and network support team can stay connected.
• ADL’s and IADL review.
• A compensative care plan for the client.

Rates and Service Terms

• Rates are based on the client's assessment evaluation and their care plan needs.
• Signed Service Agreement (Two weeks' advance notice is required to cancel the agreement).
• The minimum requirement for care includes visits three (3) days a week, with a minimum requirement of a four-hour daily shift.
• Two-week refundable service deposit required.
